1. Job Identification
Job Title: SLS (Supported Living Services) Staff
Reports to: SLS Supervisor
12 hour shifts--Mon-Sun available.
2. Job Summary
Assist adults with developmental and physical disabilities in their daily lives. Teach and coordinate adults to live and maintain an independent lifestyle. Serve as an advocate and liaison on behalf of clients.
Knowledge of developmental disabilities
Knowledge of dual diagnosis
Knowledge of behavioral issues, including aggressive, non-verbal and self-injury behaviors, knowledge of Individual Program Plans
Knowledge of ABA techniques
Knowledge of human behavior and performance
Knowledge of principles and processes for providing personal services
4. Education and Experience
High School Diploma or GED
1 year experience preferred supporting adults with developmental disabilities
5. Essential Functions
Review and follow daily routine(s) for clients' (visual and active) as written in Individual Program Plan and update Supervisor when necessary
Assist client with household chores, including laundry, light housework and bill paying
Assist client with the preparation of daily meals and assist with clean-up
Assist client on errands; doctor appointments, grocery shopping, personal errands etc.
Assist client with grooming and self-care
Provide reports on client progress as outlined in the IPP. Enter all notes into client notes system
Participate in client assessment, planning and evaluation process, and inform Supervisor of any changes
Regularly communicate with family as needed
Develop community learning (individual and group) exercises to help client reach IPP objectives
Attend regular staff meetings and participate in continuing education and trainings
Maintain positive relationship with client
Performs other related duties and assignments as required

7. Physical Requirements
The SLS Staff works both indoors and outdoors in all weather conditions. Frequent lifting to 50 lbs. plus, hearing, seeing, talking. Occasional running, walking, driving, kneeling, sitting, standing, reaching, twisting, squatting, stooping, bending, pulling, pushing, feeling, touching, talking, crouching, grasping, hearing, fingering, and repetitive motions.
8. Other Requirements
The position will require the Candidate to be at least 21 years of age (to drive for People's Care), possess a valid California Driver's License, registration and auto insurance. Candidate must successfully pass all pre-employment requirements, including but not limited to background check, physical etc. If required to drive, candidate must possess proof of valid insurance and registration.
